Staff contacts

Templates that tell you how to contact a member of staff and their next of kin. Like all other templates here, these have been gathered from those with lived experience of running a care budget. We are not legal experts and the staff contract is a very important document both to you and your staff. Read them very carefully and seek legal advice.

The idea is that you download the documents that look suitable for you, then change them so they fit what you need. You can always email us at [email protected] with your version and we'll add it.

We do not claim to be legal experts, we are people that run Personal Health Budgets (PHBs) for ourselves or for a close family members.
