Three documents

The idea is that you download the templates that look suitable for you, then change them so they fit what you need. You can then always email us at [email protected] with your version and we'll add it.

We do not claim to be legal experts, we are people that run Personal health Budgets for ourselves or for a close family member, as such this wiki is to be viewed as platform where we (and you!) can make documents publicly available for others to view and make use of it, after the documents are reviewed as being fit for purpose. ACCIDENT LOG
A bandaged hand

An accident log is an important document for health and safety. You need to know all the accidents, so at the very least you can try to work out how to stop them happening again. The accidents could happen at home, or out and about. If your staff are working for you at that time and have an accident, this log needs to be filled in.

A Person Centred Plan is a guide to the person needing care. It is more than just an 'instruction guide', it is highly personal, including likes and dislikes. It is similar to a Care and Support Plan in some ways with sections that can be included in both documents.
