This is really what it all about.

The idea is that you download the documents that look suitable for you, then change them so they fit what you need. You can always email us at [email protected] with your version and we'll add it.

We do not claim to be legal experts, we are people that run Personal Health Budgets (PHBs) for ourselves or for a close family members. ACCIDENT REPORTING POLICY	This policy outlines the procedures that are to be adopted when any employee experiences an accident, near miss or dangerous occurrence during the course of their employment. CARE AND SUPPORT PLAN	One document you will need (especially if you live in England) is a Care and Support Plan. We are thinking of ways we can help with this as everyone is so different that they really are unique. Click here to see some examples you can download.

CARE LOG	This form is a record of care. It's used for staff to look back to see what has been taking place and for you to make sure that everything is happening that should be happening. FEEDING POLICY	This policy outlines the procedures for feeding. What to cook, how to prepare and how to feed. Click here to download.
GOING TO BED POLICY	This policy outlines the procedures for getting ready for going to sleep. Click here to download.
INCIDENT REPORT	Use this when something 'bad' has happened but with no injury. It's needed so it can be recorded, examined and changes made so it doesn't happen again. Click here to download. MEDICATION POLICY	This policy outlines the procedures for administering medicine. We have a template for medicine administrating, and one for recording all health-related visits - have a look at Templates. Click here to download.
PERSON CENTRED PLAN	A Person Centred Plan is a guide to the person needing care. It is more than just an 'instruction guide', it is highly personal, including likes and dislikes. It is similar to a Care and Support Plan in some ways with sections that can be included in both documents. Click here to download.
